DuBose Care Management provides personalized services acting as a fierce and compassionate advocate and liaison to benefit individuals going through the challenges of aging. We are your support with progressive illness, home health needs, relocation, psychosocial or functional issues and crisis intervention, to name a few. Our goal is to integrate client centered solutions from simple to complex. DuBose Care Management is committed to preserving your dignity and
quality of life.

Our Story

The profession of geriatric care management was primarily influenced by my Mother who was an RN and came as a second career for me. She honored me with the privilege of caring for her as multiple debilities affected her independence. I had the good fortune of meeting a “professional geriatric care manager”during my Mother’s illness that would assist me during respite periods so that I remained strong and focused on her care. After her death I decided to leave my retail management career and pursue this amazing field of assisting individuals and families throughout the many challenges of aging.

My Mother frequently would tell me how “competent” I was and “skillful” at interfacing with her physicians and facilitating communication among all providers during multiple hospital stays. In addition she felt all her medical needs and status updates were effectively communicated to ensure her dignity and quality of life remained paramount.

Currently I have 13 years of experience in the ever changing and valuable field of geriatric care management. Most recently I have obtained my license for guardianship.

This wonderful career change has been fulfilling, challenging and heart wrenching but with each precious client and family members I have become a better care manager. I will remain true to my desire to be a fierce, compassionate and dedicated advocate!

The Professional Geriatric Care Manager

The Geriatric Care Manager (GCM) is focused on maintaining the well-being, independence and dignity of elders and dependent adults, while balancing the special needs and problems of families caring for them. All GCM members have advanced training, education and experience in gerontology, social work, nursing, counseling and/or human services.
